Walk down any grocery aisle and you’ll meet a small army of sugar substitutes — packets, syrups, “zero sugar” labels. Which ones actually help with diabetes, and which are marketing? Here’s the calm, evidence-based tour.
The three families of sweeteners
1. Artificial sweeteners
Aspartame, sucralose, saccharin, acesulfame-K. Hundreds to thousands of times sweeter than sugar, with zero or near-zero calories and little to no direct effect on blood glucose. Approved by the FDA with decades of safety review at normal intakes. The practical caveat: they keep your palate tuned to intense sweetness.
2. Plant-derived, zero-calorie
Stevia and monk fruit. Extracted from plants, essentially calorie-free, no meaningful glucose impact. Popular with people who prefer “not artificial” options. Often blended with erythritol in granulated products — check the label.
3. Sugar alcohols
Erythritol, xylitol, sorbitol, maltitol. Found in many “sugar-free” candies and bars. They contain some carbs/calories and raise glucose less than sugar — but not zero (maltitol especially can move numbers). In larger amounts they commonly cause bloating and digestive upset. Important: xylitol is toxic to dogs.
What about “natural” sugars?
Honey, agave, maple syrup, coconut sugar — nutritionally, these are still sugar. Your body treats them much like table sugar, so count them as such. “Natural” on a label is not a free pass for blood glucose.
Practical bottom line
- Swapping sugary drinks for zero-calorie versions is one of the highest-impact changes available — liquid sugar is the fastest spike there is
- Water, sparkling water and unsweetened tea/coffee remain the gold standard
- “Sugar-free” cookies and candy can still be rich in refined starch and calories — read the total carbohydrate line, not the front of the package
